COVID FORMS & INFORMATION

COVID-19 OFFICE POLICIES

  • We will not be using our brushing station. Having your child brush and floss before they arrive will give us our best ability to see how home care is going and review brushing techniques with them.

  • We are allowing only one adult to enter the office with the children that have appointments (no additional adults or children are allowed), exceptions can be made for interpreters, PCA’s and families with multiple young children.

  • All adults (one parent/guardian, one interpreter, one PCA) must be wearing a face mask or cloth face covering at all times while in the office.

  • Children must wear their mask or cloth face covering at all times except for when they are having treatment completed (exceptions can be made for children with special needs, pre-cooperative children and/or children under the age of 2).

  • Upon entering the office, everyone (adults and children) will be required to use hand sanitizer and their temperature will be taken. If anyone has a temperature of 100.0 or higher, everyone’s appointments will need to be rescheduled.

  • We will be following social distancing along with additional cleaning of frequently touched surfaces (such as door handles, bathrooms and the toy tower), and removal of objects that are hard to disinfect (such as magazines, toys and ipads).

**These forms are required before we can perform treatment.** Please fill out the form below.
